New Unit Charter Application
Please fill out this form in its entirety.
To apply for a charter from NAP, please submit the following form and supporting documents. All applications must include the following:
- List of founding members and their status (min. 5; must be current NAP members)
- List of unit officers and their term start and end dates
- Resolution of Intent to Form*
- Bylaws as Approved by the NAP Bylaws Committee
- Application signature page*
